Position: Companion / Caregiver

Supervised by:
Care Manager or Staff Coordinator

Job Description: A companion position is defined as a non-skilled/non-medical home care provider. A home care provider is a non-licensed care position that provides but not limited to companionship, personal care assistance, meal planning and preparation, housekeeping, laundry, household management, medication reminder, safety awareness assistance, exercise assistance, accompaniment to doctors appointments, accompaniment to social functions, observation and monitoring.

Responsibilities:

  • Maintain service record accurately as per agency requirement.
  • Submit timesheets on time.
  • Adhere to company's policy and guidelines.
  • Update all the necessary requirements set by the agency.
  • Regularly report client status and care progress.
  • Attend and pass mandatory in-house training program and updates.
  • Observe and report potential exposure to health hazards.
  • Strictly adhere to protect client's personal information.
  • Be able to meet standards set by the company.

Qualifications:

  • Must posses a great deal of compassion towards the elderly.
  • Must have at least six months of home care experience.
  • Must have good background references.
  • No criminal record.
  • Must have good moral character.
  • Must be reliable and trustworthy.
  • Must be able to follow written and verbal instructions.
  • Must have a good command of the English language.
  • Medically fit to work.
  • Must posses a high level of professionalism.
  • Must provide evidence of previous employment.

Position: Certified Nursing Aide (CNA)

Supervised by: Care Manager or Staff Coordinator

Job Description: A Certified Nursing Aide is a non-medical/non-licensed home care provider. The level of personal care and homemaking services are geared towards making the clients stay home and helping them perform their routine activities of daily living in a safe environment. Care provided is but not limited to companionship, personal care assistance, meal planning and preparation, light housekeeping, laundry, household management, medication reminder, safety awareness assistance, assistance in exercise, accompaniment to doctor's appointments, accompaniment to social functions, observation and monitoring.

Responsibilities:

  • Maintain service record accurately as per agency requirement.
  • Complete appropriate records.
  • Adhere to company's policy and guidelines.
  • Update all the necessary requirements set by the agency.
  • Regularly report client status and care progress.
  • Observe and report potential exposure to health hazards.
  • Strictly adhere to protect client's personal information.
  • Must be able to meet standards set by the company.
  • Report changes in client's condition.

Qualifications:

  • Must have at least six months of experience in a long-term care facility.
  • Must possess a great deal of compassion towards the elderly.
  • Have successfully completed an approved nursing assistant training program.
  • Must pass competency test to be certified as a nurse aide.
  • Must be listed in the Illinois Nursing Aide registry.
  • Must successfully undergo criminal background check.
  • Must be able to speak and understand English.
  • Must have good moral character.
  • Must be reliable and trustworthy.
  • Must posses a high level of professionalism.
  • Must provide evidence of previous employment.
  • Must have no documented findings of abuse, neglect or misappropriation of property.

Competency Skills:

  • Basic knowledge of infection control procedures.
  • Reading and recording temperatures, pulse and respiration.
  • Maintenance of a safe and healthy environment.
  • Safe transfer techniques, ambulation and exercise.
  • Range of motion and positioning.
  • Knowledge of emergency procedure and how to recognize one.
  • Observation, monitoring, reporting and documentation of care status.
  • Knowlege in adequate nutrition and fluid intake.
  • Basic communication skills.
  • Recognition of and respect for the privacy and property of the client.
  • Knowlege in appropriate and safe techniques in personal care:
    • Shower and sponge bath
    • Bed bath
    • Nail and skin care
    • Oral hygiene
    • Toileting and elimination
    • Incontinence care
