Intake Process:
Step 1 - Initial Telephone Assessment
Upon initial phone call, we take basic information regarding the client to help us select the appropriate caregiver. We then set up an appointment with a care manager to assess the case.

Step 2 - In Home Assessment and Interview
The client's condition and ability to perform activities of daily living are carefully evaluated to determine if the client is appropriate for home care and how much help is needed. The client will have the chance to meet the caregiver and can ask questions regarding the special needs and requirements of the case. The plan of care is also closely coordinated with your physician, nurse, social worker and other people involved in your care.
Step 3 - Signing the Agreement and Orientation
After careful evaluation, the plan is reviewed with responsible family members or legal guardians. Then, the service agreement is signed. The care manager will conduct extensive and detailed explanation to the caregiver regarding the case. Our staff coordinators will monitor and review the case and will suggest improvements if needed. Caregivers will document the client's care and concerns are addressed promptly. Regular visits are then made to assess the client's condition and caregiver relationship.
